Do you take insurance?
Insurance may cover some of the lab testing or provide reimbursement to you for consultation fees, but this depends on your individual insurance plan. Medicare, Medicaid and third-party insurance companies are not billed for consultations or other services provided. We are not a participating provider with Medicare and have opted out of Medicare which means your consultation or appointment costs cannot be submitted to Medicare for any coverage or reimbursement. We do accept payment from health savings accounts or flexible savings accounts (via HSA/FSA cards) and can provide a superbill that you can use to submit for reimbursement from your HSA/FSA accounts or insurance plan. Payment is expected at the time of service.
Do you offer primary care?
We are functional medicine consultants. We do not take the place of your current health care team, but rather collaborate with your existing primary care doctor and specialists. We require all patients to have and maintain a relationship with an outside primary care provider who can address emergencies, routine care and screenings.
What is required before my first appointment?
Please click on the patient portal link on the home page of the website. Once on the patient portal page, click on “New Patient Registration” and begin the new patient paperwork process. Once logged into your patient portal account, click on Questionnaires and fill out all of the listed questionnaires and consent forms. Please also list any current medications and over the counter supplements you take on a regular basis. You can also upload any pertinent consultation notes or labs drawn in the last 12 months which can be reviewed at your initial consultation.
What is the cancellation policy?
Please provide at least 48 hours notice if you wish to cancel an appointment. There will be a fee for no-shows and cancellations with less than 48 hours notice.
